About the Program

Main Purpose
The Mobile Team Coordinator is responsible for planning, coordinating, and implementing activities to achieve performance targets for the mobile team. This role works closely with the Professional Nurse (team lead) to develop strategies that reach high-risk Men who have Sex with Men (MSM) with HIV testing, treatment, PrEP, and other health care services, while ensuring clients remain engaged in care over time.

Key Performance Areas

Logistical Support, Planning, and Client Coordination

• Design in-person and digital outreach strategies to engage new and existing MSM clients and link them to the mobile unit.
• Maintain an accurate and up-to-date client cohort database.
• Coordinate client appointments with the Professional Nurse, ensuring confirmations and attendance.
• Ensure all equipment, commodities, and vehicles are available and maintained according to project protocols.

Implementation of Community-Based Activities and Mobile Clinical Services

• Safely transport outreach teams to assigned destinations.
• Provide HTS, PrEP, index testing, EPOA, and ART services as required, following organizational protocols and DOH guidelines.
• Facilitate community-based workshops, EPOA, and index testing activities as needed.

Reporting and Administrative Support

• Submit daily reports on outreach activities to the Professional Nurse (team lead).
• Ensure all clinical and programmatic data is captured and managed according to data management SOPs and guidance.
• Support administrative tasks and other activities as required.
