About the Program

Main Job Purpose
The Client Liaison Officer (CLO) is responsible for supporting clients at the Ivan Toms Centre, its mobile units, and the Call4Care line. The CLO works closely with the Project Manager to ensure client retention, referral, and linkage activities for ART, PrEP, and general health services are fully implemented according to protocol.

Key Performance Areas

Supporting Retention in Care

  • Ensure clients remain engaged in care by providing high-quality service and maintaining supportive relationships.
  • Trace and follow up with clients who are lost to care, late, or identified as high-risk.
  • Identify clients at risk of defaulting and provide targeted support to maintain their engagement.
  • Assist the Project Manager and Senior Clinical Advisor with chronic dispensing unit activities as needed.

Client Engagement and Linkage

  • Act as a primary contact for clients at the Ivan Toms Centre, mobile clinics, and Call4Care line.
  • Welcome clients professionally and link them to the appropriate services according to protocol.
  • Support the Call4Care line during business hours to facilitate timely client linkage to requested services.

Testing and Education

  • Conduct HIV counseling, testing, and HIV self-screening distribution following Department of Health and Ivan Toms Centre guidelines.
  • Provide comprehensive counseling, including couples counseling, index testing, and risk assessments tailored to client needs.
  • Manage cases for newly diagnosed clients or as requested by clinical staff, supporting ART initiation and viral load suppression.
  • Deliver HIV prevention, treatment education, STI awareness, and PrEP education.

Maintenance of Records and Reporting

  • Maintain accurate communication and case management records.
  • Submit daily or weekly reports tracking client progress, successes, and challenges.
  • Complete all required DOH, Ivan Toms, and Anova data collection and reporting tools.
  • Maintain detailed records of case management activities for each client.
  • Support administrative tasks and other duties as required by the Retention Coordinator or Project Manager.
